is it normal, for the claimant/their solicitor to have sent a blank/incomplete DQ?
Gladstones letter states its completed, but not a single box has?!

I wouldn't spend 30 seconds worrying about it, its not relevant to your defence2

The form being blank is just an indication of carelessness.
Item 7 on that checklist you were following when you filed a Defence says...

Hope we are all well
..Well I had a mediation chat with a really nice and very helpful mediator.
Their team managed to mis type my phone number. Instead of cancelling this citing their teams errors, the mediator went through the records and found my number and called me which was really nice of them.
My advice, if you have a mediation appointment, reply to the email they send, and highlight your contact details such as your mobile number / landline. You may not get a helpful person like I did

Although their solicitor mentioned their client offered me £20 to close this off, and that POPLA ruled against me, but no, I didn't offer any money.
My argument was simple enough - paraphrasing; not the driver at the time and the POC - un clear
