  • Do you think it's OK to include typed sheet answers to the questions, instead of using the small provided boxes (annotated), as my writing is poor and big?

I would suggest that probably the most important thing to remember is not to 'try and be brave' to downplay the effect the conditions have on your daily life.  You need to report the conditions as they affect you on an average or worse than average day.  I'm not saying to exaggerate the issues, but certainly don't downplay them.

Sure, thanks. It's about being self aware. I've downplayed things in the past through guilt and embarrassment and was forced to go down a 2 year tribunal route due to their cynical interpretations. I feel so sorry for people who are less cogent with language to explain their difficulties.

Yes, absolutely fine.  Just make sure your name and NI number are on every sheet.

Yes that's a very sensible way to do it.

You can review and amend it to fine tune it, and it's easily legible.  Easier for you to keep a copy for your records.

As Spoonie says put your details at the top of each sheet. Number the sheets, and make clear which question you are answering.  On the actual form write 'See attached sheets'.

Alice Holt Forest situated some 4 miles south of Farnham forms the most northerly gateway to the South Downs National Park.

Absolutely and it fits with the advice to retain electronic copy of your entries for future use. As advised refer to the additional sheets from the form where relevant 'see additional page 4' as example... and on each sheet state name, date (I would set that as ultimately the date you sign and send off the form), NiNo, page number and of course state the form section you are answering as relevant.
