Which template is best for appealing a PCN when using a lease car (Driver left site)

Hi all - I've received a PCN on the basis of "Driver left site designated for customer parking only", it was sent to me after the lease company who own the car gave them my details. The evidence page shows 4 pictures of the car with no one in it, but there is no evidence that the driver left the car-park, and even then who was driving when it parked up.
I am going to appeal on this basis (as we did visit the stores on the estate)... But which is the best template letter to use, and just saying there are no pictures of anyone leaving the site enough?
I found a few templates on the newbies page but not sure any are directly suitable. Maybe at this stage not much detail needs to go into the appeal I will keep researching, but wanted to see if anyone had something similar with a lease car (I am sure it has happened).
Also it might be worth noting, there is a 6-week gap between the parking charge date and the date of the letter to me, is this relevant?
