Re-opening help to buy ISA

My property fell through 6 months ago and I am trying to get a purchase failure notification to submit to the bank so I can reopen my help to buy ISA. How would my solicitors go about obtaining this purchase failure notification? They’ve never done it before and the bank say it’s something the solicitors needs to obtain. I’m trying to help them (even though it’s not my job) because I don’t want to miss out on my bonus as I only have another 6 months to re-open it

https://www.helptobuyisaadmin.org.uk/lib/sites/default/files/Conveyancer_Guidelines.pdf explains the rules applicable to conveyancers, including their responsibility to issue (not 'obtain') a Purchase Failure Notice (clause 8.8) and a template for doing so (Annex B ).
