Additional Permitted Subs ISA - Transfer process

Hi all. I'm in the process of setting up an APS ISA for my Mum for her to inherit my Dad's ISAs. He had two, a cash one and a stocks & shares one. 

Am I correct in thinking that his S&S ISA provider does not transfer the money directly?  So the S&S ISA provider will notify the APS ISA provider of the account value and then sell the shares and send the money raised to my Mum's bank account. Then we move the money from there to the APS account.

Does the same process apply to my Dad's cash ISA, or can the money go directly from there to the APS ISA? The providers are all different and Mum doesn't have any existing accounts with them.

    My Dad's Nationwide cash ISA, once we'd sorted probate etc, was converted to an APS ISA (inheritance ISA in Nationwide parlance) in Mum's name, which she transferred to a 1yr fixed ISA elsewhere.
