New job asking for references

I have in the past provided "to whom it may concern" references for departing staff.
In the latter case, it is not unusual to receive a call from a prospective employer who will ask "can I confirm that you wrote this reference?". You have to assume the faked reference will be verified...1 -

They are of little use in the UK but may, at least in the past, have been of some use to those relocating to a different continent.1 -
The issue here just to try and hammer it home isn't the fact you're being asked for a reference, but the fact you've given a misleading reason for finding new employment. This issue is caused by this deception, though I do understand a lot of people in the same position would be tempted to avoid the topic for fear of losing the opportunity.
I don't see how adding more deception on top of this is going to help - if avoiding the truth is as bad as it seems to be, intentionally taking actions to prevent it being discovered is even worse, and you're basically taking a hammer to the thin ice you're walking on.
We can't know what the reference will contain, but it does have to be fact. If you were dismissed, it can include that - sometimes they won't as part of policy but sometimes they will. We can't know that, but it's time to sort the situation out, and once you start being honest with your company, then and only then will things get better. If they do take it badly that's simply the result of you not being up front with them, so use this as a learning opportunity in the worst case as you really want to put this all behind you, but it can only be done when you own what has happened.

Just be honest give them the details, If they do terminate you at least you should have good references for the next time considering you have been working there for 3 months.0

No, you can't fake the reference from the previous employer - unless you provide a fake address to which the current employer writes and it would leave you open to being taken to court for obtaining money by deceit (fraud).
If you mean provide a fake testimonial well yes you could do that (in that it is physically possible to write something out and present it) but it would still be fraud.Some kind of deception offence, maybe obtaining a percunary advantage? Hope it works out for you OP, no judgement here.
Just play the game, hold your nerve, don't do anything daft and then if the new employer has issue then you can offer an alternative. Never been a fan of write my own references and playing sneaky to find out is written if you know what I mean and it has served me well.
I'm going through checks and a recent past employer personal contact didn't wish to embellish how long they'd known me for so a short time was duly noted (totally understandably) the brutal truth didn't meet criteria so a professional friend of 20 years stepped in as a character ref.
Equally I walked out of a job 10 years ago not serving enough notice as a hanging offence, paid for this at the time and got a reference from the company involved inside of the 6 years era when needed which paid off so sometimes good things can happen.
