Discretionary commission form for a deceased account holder

If I use Martin Lewis's tool for writing an email complaint about discretionary commission, and it was in my spouse's name, who is deceased, do I put her name on the form, or mine and send a copy of her death certificate?

The executor of the estate retains the right to act on behalf of the estate. So, they can put in a complaint. If the lender has never previously been notified of the death, then expectation would be that the death certificate, Will and Probate (if obtained) would be needed to be supplied.
