Question about PIP review

Hello, my wife who has been on PIP since 2014 for a long term medical condition won a tribunal in 2020 after the assessor failed her and gave her zero points. It was awarded back for 5 years at the tribunal but now they have sent a review form again. I have noticed on the form it is asking if any changes have occurred since 2016 which was 4 years before the appeal/tribunal, so they seem to have completely cut out the interruption from 19/20 where we had to go to appeal and any changes of points based on that.
So my question is, how are we supposed to fill out the "review" form? It is asking if any changes have occurred since 2016 but obviously going to a tribunal and winning the payment back in 2020 WAS a change. So surely they already have the record of this?
I don't know why they are asking for any changes from all the way back in 2016 rather than any changes since 2020 when she was awarded it after appeal.
Shouldn't it be based on her most recent award?

Fill it out as if it was a new claim.
Fully and comprehensively explain why she can't do the relevant PIP activities reliably and on the majority of days.
Enclose additional sheets (preferably typed) with detailed information covering each relevant PIP activity.
Enclose supportive medical evidence, and evidence from carers / family members who provide care confirming the care needed.
Attach a copy of the Tribunal decision, together with a note explaining if her conditions / abilities are the same / have worsened from the date of the tribunal.
I'm sure you don't want the stress of having to appeal a flawed PIP assessment again, so make your form as watertight and compelling as possible with a complete picture of her difficulties around the PIP descriptors for the DWP decision maker. Fully list all aids needed, etc

Although the tribunal outcome will be on the computer system, Im pretty sure as a PIP assessor I was advised not to look at the outcome.
Which is crazy if you apply any sense to the situation. Why assess a claimant and produce a potentially floored assessment for it to go through to tribunal again.Proud to have dealt with our debtsStarting debt 2005 £65.7K.

Plenty of changes & evidence to send in.
