OK, issued by Hospital Security staff, perhaps, unless the parking operator did their own patrols. And I assume the operator in site is Car Parking Partnership (a subsidiary of ParkingEye) but if you still work there, you can tell from the signs. Anyway, email PEye's DPO first, to get copies of the windscreen PCN (if any) and Notice to Hirer (if any) plus all photos taken and also the communication about you that they received from the NHS Lease firm. That will show you the dates and details.
Main message at this stage is:

How can the lease firm possibly have had a copy of that?
That is the third document in this chain:
1. Windscreen PCN (evidence? Was there one?)
2. Notice to Keeper (whoever is on the V5C);
3. NTH after the keeper has named the hirer.Unless the keeper is actually a third party fleet owner, who named the NHS lease team as hirer in 2022, and therefore that NTH was addressed not to you but to the NHS lease team?

My sentiments exactly! None of this makes sense. I think you could possibly be correct but my lease car team are being very elusive about all this. As you've seen, all they have sent me is the image I have attached and their response to my query of when they passed on my address to PE and if they have a record of this was: 'We had confirmation on 11/06/23 that liability had been transferred.' Can I send a SAR to the lease car team?
There may have been a windscreen notice but at that time in our hospital, the hospital staff would put windscreen notices on cars with just a simple warning inside (our hospital, like many others, has a HUGE staff parking crisis so most staff parked 'illegally' until the in/out cameras were installed and parking permits changed).
Aside from that, I don't recall receiving anything and I have no email record of it either.

I think they never transferred liability to you, then they got scared by a DCB letter this Summer. Finally they gave your data. DCB wrote to you.
You've never had a PCN then!
Wait for ParkingEye's SAR reply.
But also press the NHS Lease team:
WHY did they sit in this for a year, or is 2023 a typo by them? Ask for a copy of the transfer of liability letter they sent, who they sent it to (DCB?) and when, and a copy of the letter that caused them to panic in 2023 (DCB letter or other debt crawler). Yep its crazy!
So awaiting response from PE - i've asked them for EVERYTHING.
In the meantime I asked the lease company to send a copy of the transfer of liability letter they sent and who they sent it to? This is their response:'We don’t send a transfer of liability letter. We enter a driver’s details on the Parking Eye website so that the notice can be redirected to the responsible driver. This was completed on 23/05/2023 when we received the Parking notice.'
The parking notice they are referring to is the one I attached a copy of in my above post.

Ask your lease company to provide strict proof of how they "knew" the details of the "driver" at the time of the PCN. You may want to advise them that they should provide a contact who can be served with court documents if they intend to stand by the allegation that they "knew" the drivers details on the date.2

And ask if they actually mean 2023, or 2022? If they mean the former, 2023, where's the letter they received that caused them to suddenly do this a whole year later than the PCN, and you want a copy of that letter they received, too.
