Completing C1 form

My C1 form was rejected today for the 4th time. My dad died last April and its still not resolved. Apparently even though I was previously told an abbreviated extract of death was sufficient, today its a full copy of the death certificate.
Also, his address on the will differs from the care home where he ended up. Do I tie this together on the form somewhere?0 -
You did say previously that your father's address was as shown in the will. If he was permanently in the care home, the care home is the address entered on page 1 of the C1.
In the first box of para 1 on page 2 enter [full name] residing sometime at [enter will address] and latterly at [care home address]
In the para 2 declaration include. In said will the said deceased is designed as residing at [insert will address] a former address.

364 days after my father died the confirmation of estate form was eventually okay'd. Now to get the bank to release the money.

Hi newbie to the forum here and i have just had my C1 application rejected for similar issues.
My late father passed away in a nursing home where he had been residing for 18 months and the address on the will is his previous home address. There are two executors named in the will, namely myself and my brother. Can quite seem to get the wording correct any help appreciated.0 -

I have attached a redacted copy of whole of page 2.
Basically there are 2 people named on the will as executors, myself and my brother.
The deceased died in a care home after being there for 18 months and the will has his old home address on it.
Indication from the sheriff clerk is problem is in the declaration but obviously would not be specific.

Suggestion
If you have shown the care home as father’s address on page 1.
For para 1 on page 2
[full name] residing sometime at [will address] and latterly at [care home address]
the Sheriffdom of Tayside, Central and Fife in Scotland
For para 2 on page 2,
That I am
along with [insert brother’s full name] residing at [insert brother’s full address] the executor-nominate of the deceased conform to his will dated the [insert date of will] which is exhibited, docquetted and signed by me as relative hereto, and that I and the said [insert brother’s full name] are sons of the said deceased. In said will the said deceased is designed as residing at [insert will address], a former address.
This assumes that there are no other name or address differences in the will.

This assumes that there are no other name or address differences in the will.
For para 1 on page 2 - that will be quite a lot to squeeze in but i can handwrite it as suggested in a previous reply to another poster i read somewhere.
For para 2 page 2 -So the brother full address needs to go in here even though it is already entered on page 1 as being an executor ?
