Switch to BT Social tariff. Not an existing customer.

I think my mum in law should be eligible for BT's social tariff.  She's currently with Now TV for her phone & broadband on a standard tariff.  However,  I can't see how to switch from Now TV to the BT one.   I've tried various links but they all take me to a page with says it's open to new & existing customers but only tells existing customers how to apply.
Most of the pages about BT's social tariff talk about an online form which I can't see any sign of.  Filling in an online form would be a lot easier than phoning up and having to pass the phone backwards and forwards to explain things!
Does anyone know how to apply please?
Now Tv do offer a social tariff but they don't bundle a calls package like BT does.
