Broadband choices voucher email

I signed up to Shell broadband 4 months ago, via Broadband choices website
They said I'd get an email with the £105 voucher, then have 3 months to claim.
What address will it come from?
Is there a custermer service email/phone contact, that people know of?
Has anybody successfully claimed it?

I think the vouchers are provided by a third party company.
At least that was what happened with BT a while back.
All sorts of excuses given by BT for me not getting my voucher.
Never did get it but BT gave me a credit to my account instead.
Which was better than the voucher to be honest.
