Second Adult Rebate form question, any ideas ?

I currently get IR ESA and I live with my elderly mother. She currently pays full council tax and I am fairly sure I can get her a 25% discount. I went online to fill in the "Council Tax Support" form, is this the correct one to fill in for Second Adult Rebate ? As I can't see one specifically titled Second Adult Rebate. I am not looking for housing benefit, just the discount for 2 adults in a home with secondary one on benefits.
While filling that in I got a bit stuck on the adding another person section, please see attached screenshot of what I mean. When it says "Type of income" do I put in "Employment and Support Allowance (ESA)" in this case ? Secondly where it says amount, is this the amount of benefits I get per week or fortnightly or monthly ? Or do I just add "weekly" or "monthly" beside the amount ? I am paid fortnightly.
Does my mothers savings make any difference to Second Adult Rebate reduction ? Also do I need to send in any evidence of any kind of my benefits ?

Yes that is the correct form, yes put ESA IR and the amount plus frequency.
The LA will in all probability ask for proof of your ESA.
The savings shouldn't matter because you are looking to qualify for a discount through your income and not your mothers financial circumstances. But each LA has different rules for Council Tax Support.
