Ofcom - back billing rules

I cannot appear to find the Ofcom rules pertaining to "back billing" similar to those in place for Ofgem.
Can anyone at all point me in the right direction.
My elderly relative has been undercharged for a mobile phone contract for over a year and this only came to light when I has asked whether the tariff they were on was still competitive. They are now on a standard tariff however the discounts applied look to be duplicated and/or incorrectly applied and the monthly charge amounts to only £1.08 despite having unlimited calls and texts.
Can anyone at all point me in the right direction.
