UC is based on your overall individual circumstances to form Total Entitlement and are different for everyone.

    Age
    Single/Joint rate
    Children
Rent - whether private / social (Local Housing Allowance dependant on circumstances)
    Childcare
    Carer
    Disabilities

    Deducted from Total Entitlement

Work Allowance if Children/LCWRA - before deduction of 55% of earned income over work allowance.

If no Work Allowance, 55% deduction of earned income entirely.

Deductions are also made to pay back any Advances / Debts.

Hence your Award could be very different from your Total Entitlement.

Disability for UC means a health condition that limits your ability to work, as confirmed by a health assessment.  So unless the effects of your diabetes severely limit you, probably not.

The criteria are here https://www.entitledto.co.uk/help/esa_criteria  You'd have to score 15 points from the first section (which would give you the work allowance but no extra money) or meet one of the criteria from the second section (which would give you the work allowance and more money).

Will get the 1.5k rent and help with service charge

The maximum amount of help you can get with rent and service charges is capped and depends on your postcode and the number of bedrooms you are entitled to based on how your household is made up.
    https://lha-direct.voa.gov.uk/Search.aspx
the amounts shown on the above website Is this what I would get a week to help with my rent?
As advised previously, it's the maximum amount you could help to help with rent.

When you claimed UC did you report having a share of the property you own? If you didn't then you will need to do that. If your share is more than £16,000 then there will be no entitlement to UC. Whether there's any entitlement anyway will depend on your circumstances.

I read it as the poster is living in the shared ownership unit in which case the capital value is ignored.
There they have now moved out, there way be a capital disregard for a period depending on the details.
For would certainly help if they provided information at the outset!

your posts are a little confusing

there appears to be two situations

you currently/recently lived in a jointly owned, shared ownership property with you ex
you are looking to purchase another shared ownership property for yourself

If the scenario above relates to your ex then the first question would be whether they are able to get a single mortgage/buy you out to see whether it can go to single ownership or whether the property will need to be sold or you need to remain co-owner and on mortgage
