Anyone know how to set up a second hand Lenovo ThinkPad.  Just got it today as a temporary replacement for a Mac but can't set it up.  Anyone have knowledge of this.  I'm only getting the 'setup page' but can't figure out how to get moving and get the start menu up.  The format of this looks very old fashioned.

Sounds like the computer has booted into the BIOS environment from the description, probably because there is no operating system eg Windows or no hard disk.

When you bought the Thinkpad was it advertised as having Windows 10 installed?  If it is installed on the hard disk then you need to hit one of the Function keys (try F12) when it's first switched on.  That should take you to a menu where you can select the hard disk or it may give you the option to restore or repair the installation of Windows.  If it's not installed then you need to buy Windows and install it from a drive installed/attached to the laptop (this could be a USB).

Yes it has Windows 10 installed (ex NHS machine).   Trying to do as you suggest with F12.  Got a menu up - do you know which section the hard disk option will be in. ie Config, security, start up, restart, main ?

Without knowing how the seller reset the machine it's hard to say which you need. This article describes a process for reinstalling Windows on a Lenovo machine which has been factory reset.

ETA If they've done anything other than reset the laptop in accordance with manufacturers instructions I would expect them to give you some documentation that explains how to restore Windows.  So if you can find the online manual for your model you may find it's really straightforward

We can't get on to the start menu page to start this process unfortunately.
