Along with printing the statement take a screenshot on a PC if that includes your name and address and print that as well.

If there is a DOWNLOAD option that may include all the details you need.

Press prt sc  (Print Screen) and paste into your usual image application.

prt sc is usually on the top row of keys towards the right next to the delete key.

Pressing prt sc takes an image of the entire screen.  You can then paste it into Paint or whatever image app you use and then edit it for size and print it or send it via email.

Edit: Snip and Sketch as described below may be a slightly better or faster method than prt sc.
A man walked into a car showroom.
He said to the salesman, “My wife would like to talk to you about the Volkswagen Golf in the showroom window.”
Salesman said, “We haven't got a Volkswagen Golf in the showroom window.”
The man replied, “You have now mate".

Snipping also works on W10 using Snip and Sketch.

I spent an hour or so doing just that for various documents for a visa application where I needed to submit everything in small (<3 Mb) jpg files.

As a Windows user since 3.n can I just say I still don't understand the snipping tool.

Thanks for that. :)  I haven't used it before as on the very rare occasions I need a screenshot I use prt sc.

Snip and Sketch may be a bit faster than prt sc if you want to resize the image.

I have added Snip and Sketch to my taskbar.
