I wasn't going to change the heading as its a witness statement and not a defence. If putting the same heading as the defence is the correct way then I'll change it.

On your version, top left will say

IN THE COUNTY COURT AT XXXXXXXX

(like your costs assessment does).

Costs assessment isn't an exhibit so remove the exhibit number but do list it on your cover page.

Not sure which exemplar the OP used but the two I believe used currently (_blueberry_ and vincentvega27) both have "Hidden Terms" heading and other paras not used in this WS.  

After para numbered 34 it continues then with another #20 sequentially.

So the Witness statement was submitted this afternoon and a response was received from DCBL. They have informed me that they have already submitted a  notice of discontinuance. They have attached a copy for my records.

I'm pleased that it's over, but it just feels a little bit of an anti climax after compiling the WS. I'm sure I'll get over it.
