Another Santander question

I want to transfer my existing Shawbrook ISA into a Santander ISA, I am not sure how to answer the questions in this section of the form, I want to transfer the whole ammount, I haven't paid into the ISA in the current tax year. Does anyone know how I answer Questions 1&2? Thank you for reading this and sorry if it is a stupid question.

Answering yes to the "Do you wish to close your existing (non-Santander) cash ISA and transfer all the balance..." question means that you should not answer (1) and (2).
