Tui - Flight Delay Compensation Form - Not working?

Hi, successfully submitted a form lastweek and 2 out the 5 of us were rejected as not on manifest. Have to submit a new form but lo and behold the form is not working?
Has anyone had any issues with the Date of Birth field? Has anyone successfully submitted a new form in last few days? If so what web browser?
i have tried tweeting, chat, facebook but no joy. Thanks
