Help with RX3 form

We have a restriction on our deeds which we are attempting to remove.
Spoke with Land Registry who advise to fill in the form RX3.
Tried to call them but their call queues are very long so i cant get through to them.
Could anyone advise if the below sounds correct (Their questions in italic and my question/answers in bold)
7 Documents lodged with this form: - What proof could i send? Im not sure if this is from me or the person that added the restriction in the first place?

The applicant applies to cancel the restriction registered on
[A] in favour of [B]
against the title number(s) listed in panel 2. The application relates to
1 the registered estate
0 the part(s) of the registered estate as shown:
0 the registered charge dated in favour of: [C]
[A] - There are a few dates on the deed and Declaration of Trust, how would i know which date?
[B] - Should this be the current mortgage payers (who are also listed on the DoT/deeds)
[C] - Again not sure where i find the "Charge" date?
9 State why the restriction is no longer required: - One person is moving out and purchasing their own place - would this suffice?[A] - There are a few dates on the deed and Declaration of Trust, how would i know which date?

I haven't seen those videos so will give them a watch soon.
The restriction is
No disposition or dealing by the proprietors of the registered estate is to be registered unless one or more of them makes. a statutory declaration or statement of truth, or their conveyancer gives a certificate, that the disposition or dealing is in accordance with trusts contained in a Declaration of Trust0
