If you had the form sent to you it is date stamped in the front page with two dates. The first date is the claim start date (the date you rang) provided the form is returned by the second date (6 weeks after the first).

Hi all, just an update. Still no letter about the award, so husband rang them yesterday. They're sending a letter out and said it will take 4 to 5 days until we get it. The chap who he was talking to said it wouldn't be the decision letter, it will be proof of entitlement.
Can we ask for a copy of the decision letter? It's obvious it's gone astray as a hospital appointment wasn't received through the post. I'm not sure we need a decision letter so any help will be much appreciated. TIA
