Yes, you are right. The form was updated on 1st June.

It looks like a new Section D has been inserted and the old Section D has now become Section E.

The new Section D consists of one question...

There is also a new question in Section E...

So what is the answer to suitability for determination without a hearing? If no what is the reason?

Hi All,
I have received a letter from Gladstone today that “ the letter enclosed a copy of completed Directions questionnaire”, but I found the attachment DQ is not filled in at all. I also checked on MCOL  status, it stated “ DQ filed by claimant on 14/02/2025”. 

Should I reported the incomplete DQ to Gladstones and the small court ? Many thanks.

YOUR own blank DQ will come from CNBC.  When MCOL status changes to "DQ sent to defendant" you can download one and complete it for sending to CNBC and claimant by e-mail.
