Our setup is exactly the same as this, the fitting comes with that hole made in it as you can see from the attached photo. Ours have the wastes dropping down from above and the pipes are clamped to the fittings. Ours are a dishwasher and a washing machine. We avoid having them empty at the same time, not sure if it would be a problem or not but not risking it.

The hole is not made by a clown, it is there as a syphon breaker.
The problem in this case as has already been pointed out is that the stand pipe does not go high enough, so the hydrostatic pressure from the drain pump of either wm or dishwasher will push water out through that vent hole.
I suggest the OP buys this alternative McAlpine appliance trap from Screwfix and just adds a suitable length of 40mm pipe, open at the top, to the centre connection

Almost certainly as FreeBear says - that standpipe is not tall enough/has too large a U-bend.If you look at the standing water level in that U-bend, it should be obvious that any new waste water coming down from either the W/M or D/W will (a) have only a short upper length of pipe to fill before it overflows, and (b) have to shift quite a sizeable amount of U-bend water first, or else it'll overflow - so back to (a).I'm guessing eh correct solution, then, is to increase the height of that upper pipe before the McAlpine goes on it, and/or make that U-bend much shallower. The first is the one to try, er, first.

Do make sure the waste pipe is clear all the way to the drain or wherever it goes.Sorry I can't think of anything profound, clever or witty to write here.1 -
Look up the install instructions for that model - it'll mention the min height the hose has to be before it enters the pipe. As NSG says, I'd get the pipe up to that height before doing anything else.
