They could be annoyed if you simply didn't turn up but you will be giving them some notice.  Then they find a time when you can attend.

Did the appointment time come from your employer or from the OH service? I have people on my team who have done OH and I never know when the appointment is, the OH service arranges it all. The only time HR get notified is if someone doesn’t show up because then we get billed for the missed appointment. The OH service and your employer doesn’t know what you have planned next week, if you had something booked you couldn’t move would you feel guilty?

The employer knows when their staff is on A/L, no.

It’s unlikely then that your employer even knows exactly when the appointment is happening and so won’t be any the wiser if you push it back a week. The OH company probably just allocated you the next available time in their books, without realising you are on leave. Just let them know you can’t make that time and ask to reschedule.

While your employer knows you have annual leave they don’t know what you have planned that time, you could have a family party, theatre tickets, be staying in a cottage in the depths of the country with no phone signal… any number of things that prevent you having a phone consultation.
