There are specialist policies you can buy, though not from UK insurers and so a little "buyer be warned", that are able to be bought when you are already overseas but coverage tends to be weak outside of medical plus no Financial Ombudsman support etc if things go wrong.
The only obvious gap is medivac/repatriation so if one of you became seriously ill/injured you'd need to stay in Spain until well enough to travel on a normal commercial flight whereas travel insurance may look to bring you back to the UK by special charter with doctors/equipment etc when you are still ill but well enough to be moved.LindaJL said:

We live in our own house, flights are negligible, we have the new GHIC card and even golf insurance! For the OP - what risks are you actually wanting covered? Given you are spending 6 months a year there presumably you have local property insurance, you say you just have budget flights, EHIC/GHIC gives you medical access in line with a Spanish person etc.

If this is important will depend on your family/friends situation etc but many do want to be near those when ill.
Wonder what happens if you overstay your permitted time in Spain due to being stuck in hospital for example. And would your partner be required to go back to the UK leaving you on your own?
You'd normally be applying for an emergency visa on compassionate grounds though each country will have its own rules, exact name and how likely they'll allow the accompanying spouse/friend etc... not many countries are going to kick someone out the country where medical advice is they cannot be moved.1 -
If you wanted to have private cover you could look at a Spanish private policy?0
