Day 5 test to release & passenger locator form

Hi, .my teenage daughter is currently on holiday in Greece. She has only had one covid jab so far so has to quarantine on return.
However she is due to take an exam on day 8 of her return, that can't be changed to a different date (unfortunate but a consequence of them having flights cancelled 3 times) so we have paid for day 5 test to release. We are taking her to a testing centre to do this. As we were looking to purchase the day 5 test, we did see something about she has to fill in an additional PLF?? We couldn't find any more details though to let her know she needs to fill in this additional info.

I am only speaking from experience and presuming nothing has changed with the form since end of June but there was nothing in mine to say you were doing Test to Release and I didn't need an additional form. I did Test to Release with no problems. The Day 2 & 8 are the important ones, particularly the Day 2.
Again, this was four weeks ago. I'm not sure if anything's changed and I personally haven't heard about an additional form.

It’s all on the same Passenger Locator form - if you have already completed it, just update it with the additional details.Be careful which provider you select for your Test to release as you can’t end your quarantine until you get the results back . Which as some have reported might take a few days. ( see posts below ) which could mean your daughter would miss the exam anyway.0

She won't need an additional PLF, but will need to insert TTR details onto her PLF unless she's already filed one and will need to self-isolate until the results come back. I recommend a morning day 5 test with Collinson for something like this, as they should have it back by the evening.

Hi, thanks for replies. Hasn't filled PLF in yet, can't be done till 48 hours before return I *think* That's good that she only needs to fill her day 5 info on the same form.
Yes, we're aware she can't end quarantine until day 5 result is back.
We have paid for her to go to an approved testing centre to get the day 5 test done, so to get the test result back quicker rather than put in the post and wait for the update, which indeed would be touch and go for her to take the exam. The appointment is in the morning so hopefully the result will be back that night or the very early hours of the next morning (12-16 hours turn around is given) . Even assuming 16 hours for result. That gives well over 48 hours before exam takes place.
@CKhalvashi - Do you mean the correct box on the PLF? If so, yes that would be helpful.
If you mean the correct collinson box then thank you but no need she's already booked in to take the day 5 test.
