What do I put in question 17-20 it I have not seen another healthcare pro for many years?

It's the question regarding HCP's carers, friends or relatives. It's basically someone that they can contact if needed (although this is very rare for them to do this) If you see your GP then put their name down.

It's your decision who you put down. I usually put my GP down and in 7 years (3 reviews) he's only ever been contacted once. I put my daughters GP down too for her recent work capability assessment form and he wasn't contacted either.

No, contacted once out of 3 reviews for me. Not contacted at all for my daughters first WCA which was just a couple of weeks ago and the decision was made on her's on 6th July.

For myself yes but not for my daughter, i added an additional contact name for her but no one was contacted. My daughters was Universal Credit but it's exactly the same process and the form is UC50 rather than ESA50. If they contact a GP then they are sent a simple tick box form to fill in and return.Have you looked at the 2nd link i prvided above? It will advise you how to fill out the form.0
