Help! Which Land Registry Form to Use?

My partner and I separated last year. I have continued to live in the property we shared and although only his name is on the title deeds, I have contributed a large lump sum to the property when we had building work done. When we separated, we had a Declaration of Trust drawn up that confirms my beneficial interest in the property, ie I am entitled to 50% of the equity when the property sells.

I need to inform the Land Registry of this arrangement so I completed form SEV and sent it along with the Declaration of Trust. They have replied to say that this is not the correct form to use but I'm now not sure which form I should use! Can anyone please advise? TIA

    You can’t use form SEV if there’s just one registered legal owner. You’ll need to consider how best to protect your beneficial interest, namely your 50% of the equity value
    Our PGs 24 and 19 assist but will rely on your ex-partner agreeing/applying as appropriate. 
    Ultimately a form A restriction may be sufficient (PG 24) or another form of restriction (PG 19) which then requires your consent should he want to sell/mortgage
    But we can’t advise you on which so it’s important to understand what your arrangement means in law and what, if anything, is the best way to protect that on the register. It all sounds simple but in practice it can be complicated and both understanding what options you have and their effect is important
