I'd say that after a year the elderly couple might be nervous about travelling or they might be desperate to do something normal. Either way I'd ask them what they'd like and fall in with their wishes.

I think you need to do it now in case you need to book accommodation.
  • I feel for you OP, hope it all works out.

An executive transfer or chauffeur driven car of some sort, seems an excellent idea, especially if you specify vaccinated driver in mask , freshly valeted car and their favourite radio station.

Comfort breaks could be a much bigger problem, Thousands of people will use motorway ervices and the hygiene standards pre-pandemic, weren't always good. However, there is a book Sawday's The Extra Mile, which gives details of farm shop cafes, hotels and pubs very close to motorways. The Extra Mile – The Extra Mile Your relatives might feel more relaxed with stops at such places. Can't guarantee hygiene standards, but probably fewer people.

You could find somewhere equidistant from both homes and do an overnight stay so you all get a break. If you can stretch to it, what about two nights ?

Is it not easier for you ( whatever’s age you are) to travel to your 90+ relatives? The choice then for you is either you drive yourselves or get a taxi.
I think it’s unfair to ask elderly relatives to do the schlepping.
Life is like a bath, the longer you are in it the more wrinkly you become.

That's true, but I see conflicting possibilities ...
    • elderly relatives may be suffering cabin fever and desperate to get out of the house / to a different area

    • if you have any concerns about how well they are coping, you'll get a better idea by going to them (even before restrictions are lifted, providing care / support allows you to enter their house)
