Find your diagram confusing. Assume that's a shower at the door end while your swap arrows refer to sink and toilet. If correct then seems you have to clamber over the shower to access the toilet!

Another space-saver that could work is a corner toilet, which fits with the cistern in the corner and comes out at 45 degrees, so your leg room would be using the diagonal. This sort of thing: Laos Close Coupled Corner Toilet and Seat | Appliances Direct0
