Light it from another angle or put a straight edge on the wall to rule out shadows. With water based paints stains can continue to come through regardless of how many coats are used. You might need to seal that area with a stain blocker.

I don't think there's any discoluration coming through, it's simply the shadow of the uneven wall showing up. OP, you're not going to get rid of that by sanding down, you can minimize it with some filler but to get rid of it fully you'd need to re-skim the wall so it's perfectly flat.

As mentioned take a straight-edge, such as large spirit-level to see just how uneven the wall is.

It certainly looks uneven; when I first saw the piccies I thought you were trying to paint over lining paper.  Definitely needs sanding down.

How much do I sand it down though as I've done one side of it and it's like a very smooth ridge (not sure if that's making it look shadowed)?
