Flashed by camera, no follow-up

I hear a lot of people say that they've been flashed by a speed camera only to not get any follow-up paperwork. Why is this?
Now cameras no longer use film, surely there's no excuse for the driver to not receive a ticket. Or do cameras fire a warning flash if the car is going a little over the limit?
