(the info on their website does confirm that this is a requirement t for children too)
Maybe this was an issue with the website but it does appear that there is nothing that can be done on day of travel so it needed to be resolved the day before travel to avoid being refused travel."We wanted to let you know that the Government of Greece introduced online Public Health Forms, which must be completed prior to travel.
Please visit https://travel.gov.gr/#/form and complete the online application no later than 10pm UK time prior to you travelling to Greece. This includes all passengers and children of all ages. This link works best on Google Chrome.
You’ll get a QR code the morning of travel and you must show this before you’re allowed on the plane.
The questionnaire cannot be completed on the day of your flight and failure to provide the confirmation of application before boarding will result in you being refused travel."
It does seem you are not alone https://www.tripadvisor.co.uk/ShowTopic-g189398-i192-k13354976-Plf_for_minors-Greece.html
This is what TUI say for their passengers
- Everyone over the age of 18 must complete a separate form. Children/Infants under the age of 18 can be included on ONE of their parent/guardian’s forms."

I've read the advice above , it's very clear about each person "including children" needing a form
Visa's etc are always the responsibility of the customer , not jet2 , it's a bit like blaming them for finding out you need an esta to enter the USA and have failed to get oneEx forum ambassador

Entry requirements etc aren’t Jet2’s problem, they’re just forwarding on the information from the Greek government.Contact your insurance company, as this will be an insurance claim rather than anything to do with Jet2.Edit: I’ve just skipped through the forms, and it all seems to be there - is it possible you made a mistake?1

The Greek Government has today banned Wizzair from landing in Greece due to the airline failing to comply with their PLF form procedures.
I know this doesn't necessarily help the OP's issue, but it is an example of how seriously the Greek Authorities are taking their regulations, and why Jet2 would be so thorough.1 -

The airline urges passengers to correctly fill the Passenger Locator Forms and present them when boarding.

Although TUI say a separate form is required for all over 18s, the Greek website says:The Passenger Locator Form (PLF) is a key element in the planning. All travelers must complete their PLF until the day before entering the country, providing detailed information on their point of departure, the duration of previous stays in other countries, and the address of their stay while in Greece. In case of multiple stays, they are required to provide the address for the first 24 hours at least. One PLF should be submitted per family.

A shot across the bows from the Greek Government to Wizzair and a warning to all the other airlines.0 -

We got to the section where you add family members and after I added myself on my partners form it did not give me an option to add the children. Once we got to the airport they refused us to fly, there was another family at the side of us who had done the same thing and apparently this is happening quite a lot! We was sent home with a piece of paper with a number to ring which is very hard to get through,We haven’t heard a single thing about it from Jet2 in fact they had the cheek this morning to send us transfer details for, what would be, our return home! Nearly £4000 they had off us and not one bit of correspondence about our holiday. We are absolutely gutted and our children were so upset! I just wondered if anyone else has experienced the same thing and what has happened about your holiday?
Sorry. Jet 2 are not to blame for forms filled out incorrectly.
