They may change your commitments but there's no guarantee. The best thing to do really is to send in the fit notes. If your new GP refuses to give you the prescriptor for the acne treatment then just see another GP at the practice.

Yes tailored commitments possibly requiring you to search and apply for work, but not for 35 hours a week.  No WCA as no fit note.
I think you need to register with a GP local to where you live and they will contact your previous GP for your records.  You never know when you need help and therefore registering will enable easier access the local GP services.
You are making presumptions about the local GP's and what they would prescribe, without any reason to hold those presumptions.  And even if they did not support current medications, they may prescribe an alternative which actually works better.
The comments I post are personal opinion. Always refer to official information sources before relying on internet forums. If you have a problem with any organisation, enter into their official complaints process at the earliest opportunity, as sometimes complaints have to be started within a certain time frame.

I'm guessing that if I declared my conditions on the UC form without providing Fit Notes, and explained to the Work Coach how they affect me, I might get tailored commitments but no WCA?

At least that would be a compromise.
Work coaches have discretion to turn off or modify commitments temporarily - for instance for a period of personal stress, bereavement, temporary illness, during the wait for a WCA, etc. If you have no intention of being referred for a WCA because you won't ask for a fit note, there's not much they can realistically do for you within the rules.
