    I think we need to know the exact date we passed the test as the insurance application forms ask for it and I suppose the insurance company would use this as an excuse not to pay out in the event of an accident.

    You don't have a middle name...

    The first two characters of the last block are your first two initials.

    The DoB is "mangled up" consistently (and not particularly much)... Y1M1M2D1D2Y2. For women, M1 has 5 added.

    The "mangled"  DOB also contains a gender indicator. The alphanumerics are not random.
