Please help with c/c fraud

Thanks for reading my post.Im ina desperate situation and would really appreciate any help!I have mobility issues and this situation ruined my mental health and i cant find a way out of it.No service could access to help me,Action fraud done nothing and bank made me liable for purchases around £3500 on my credit card.
in June Lats year when my card stopped working rand the company and been asked  about few transactions made few days earlier.i rang the bank number of time and identified myself with my own address.Bank refused refund telling me my pin was used and rejected my complain.Its months in OmduSman now and i only heard from them last friday telling me the same.
IN AUGUST LAST YEAR 2 MONTHS AFTER FRAUD HAPPENED I FIND UNKNOWN ADDRESS TO ME ON MY CREDIT FILE AND REPORT IT TO TRANS UNION FOR CORRECTION.INFORMED THE BANK BUT THEY REFUSED TO TALK.I HAVE NOTHING WITH THOSE PURCHASES AND IN FACT IVE BEEN IN ANOTHER LOCATION AND HAVE FEW RECEIPTS FOR PURCHASES AROUND HOTEL I WAS STAYING THAT DAY..CREDIT REPORT SHOWING CLEARLY DAYS BEFORE FRAUD ADDRESS CHANGE USING C/C WITH THE COMPANY IN DISPUTE.POLICE DID NOT ACT DESPITE HAVING ADDRESS.
I LOST A CARD FEW WEEKS BEFORE AND WAS REPORTED WITHING 5 MIN.HOW SOMEONE POSIBLY COULD ORDER ANOTHER CARD IN MY NAME OR CHANGE ADDRESS ?MIGHT NOT HAVE WITH THE ANOTHER ADDRESS ANYTHING BUT HOW THIS HAPPENED?ONLY COMPANY SUPPLY CREDIT AGENCY WITH ADDRESS CHANGES AND MY BANK WONT GIVE ME ANY ANSWER.
MY DETAILS COULD ONLY BEEN STOLEN SOMEHOW.
SINCE LAST YEAR IM DESPERATELY SEARCHING SOME HELP AS CANT AFFORD SOLICITOR TO PAY.IM UNEPLOYED AT THE MOMENT DUE TO DISABILITY AND RANG POLICE E NUMBER OF TIMES BUT NO INVESTIGATION DONE.CANT GET TROUGH CITIZENS ADVICE FOR MONTHS AS THEY ARE BUSY.OMDUSMAN DOING INVESTIGATION BUT JUST KEEP SAYING PIN HAS BEEN USED.NO ONE WANT TO EXPLAIN HOW ANOTHER ADDRESS APPEAR ON MY FILE AND HOW IS LINKED TO THAT CARD.CARD WERE REPLACES BEFORE AND AFTER FRAUD WITH THE SAME NUMBER AND ISSUE DATE IN SPACE OF  MONTHS FOR SOME REASON.
MY MENTAL HEALTH IS RUINED AND I HAD TWO ATTEMPTS AS THIS HAS I HUGE IMPACT ON MY LIFE AND I HAVE NO HELP FROM INSTITUTIONS AT ALL.

    Your post is quite difficult to follow.  Do you have someone who could help with providing details to Action Fraud and the Ombudsman, in case they've misunderstood your complaint?

    absolutely no help,Action fraud doing nothing after 12 months and i keep them chasing.could not come to talk to anyone so far and every day im calling citizens services and cant get appointment.same with lawaid

    At the moment your post doesn't seem like it's in the correct time line order, and isn't making much sense, don't worry - as you've probably written all that while feeling quite stressed - sit down with a piece of paper - and write it all out in date order - including approximate dates - making sure each line happened after the last line in the time it actually happened, so something like:

    Jan 1st Lost my card (how did you lose your card? How did you know it was lost?)

    Jan 1st reported the card as lost to my bank

    Feb 1st - tried to use my card - didn't work - called bank - they asked about some transactions, I said I didn't recognise them

    Then copy it in here with spaces between each event - providing as much information as you can (while staying anonymous) so that we can try to understand what's gone wrong, and suggest ways of putting it right.

    If there's any reason you've been negligent in the way you've managed your account - there's not much anyone can do - but with a clearer picture of what's happened we might be able to point you in the right direction.

    1) i lost Halifax c/c early May 19 ,running on rain from the station home and was reported within 5 min.
    2)card was replaced again and again number of times with same issue date and number(even card replaced in june,aug,sep octo had the same number and issue 05/19 what was strange to people in halifax brunch.
    3)fraud happened on 7th /06 and bank investigated and sad my pin is used im responsible.i had to make number of calls and been asked many times to confirm my address as they normaly do.
    4)checking my credit file en of july i find unknown address on my file and report it to action fraud again,halifax .date of change 31/05/19 linked to halifax card.
    (people from scf and my other bank wanted to help giving me advice.creditor changed your address on your or someones request )i cant get details from the bank and they dont want to cooperate
    HOW SOMEONE POSSIBLY CAN DO THAT?HOW STAFF COULD TALK TO ME AND IDENTIFYING ME WHEN ANOTHER ADDRESS WAS ON FILE?OVERSEEN THAT IMPORTANT THING
    5) case passed to Omdusman and they keep telling me the same.
    Honestly nothing that i have done or have any link to it,unless was stolen from me in some way.never had anywhere writen my pin or card details.
    but can only link lost card to that if was used to obtain another card or another way but in that address is a clue and nobody want to go  back now and see real facts

    One thing you should do if you haven't already is to submit a Subject Access Request under the Data Protection Act, asking Halifax to disclose all data they hold about you, including, but not limited to, all cards issued and all contact (via any channel, written, on phone or in branch), especially relating to address changes.  They're legally obliged to supply this data to you and can't refuse to cooperate....

    thats amazing nobody told me this!i spoke to mny finantial institutions as hard to belive in this technology era thy cant provide me with anything.in fact they refuse to talk after claim was rejected.
    this is obviesly bank fault,they even told me tried to contact me when purchases made but was an old phone number on thir file.my bus fare1,5£ were declined and transaction gone trough for 3500£
    i havent done this and live in a fear that someone can use my identity again.this is identity teft if someone ordered another card in my name

    As per above to help with what happened.
    A few questions. Which should have been asked by the bank.
    But if you lost the card. How did someone get the PIN? That is the real crux of the matter. Please do not take this the wrong way (it is often the case) does anyone else know your pin? Do you give your card out to Family or a Carer to do shopping? Where is your card kept? Who opens your mail? Shared post box?
    Given you clearly got the new card the change of address is not relevant at that point.
    Was the address changed with the bank?
    You can only have one valid card on a credit card (unless you have additional card holders. Which does not seem the point here) so if someone had changed address at bank and ordered a new card/pin. Yours would stop working and the other spending you quote would not have happened.
    Change at credit ref, could simply be ID theft, which will not be related to card fraud, if it was a card you already held. 
    TBH. Action Fraud will only report it to your local police. It is then up to them to send someone out if they feel it is worth it.

    Are you saying that you are in possession of four replacement cards all with the same long number and bearing the same issue date of 05/19, but that they were sent to you in June, August, September and October 2019?
