    The problem is somewhere along the line you started eating more than you probably actually needed, this sets up a vicious circle. where you think you need more than you actually do to stop being hungry. 500gm of mince for a spag bol for one is quite frankly ridiculous, "Mrs" lincroft cooks 250gm for both of us and we are big eaters! That amount of red meat is not good for one's health, I have had recent health problems, and in both cases have been advised to reduce my weight slightly, exercise more and keep an eye on the ingredients in the food I eat and reduce portion sizes. 

    Unless there was an unacceptably long wait between courses, why did it take you 2 hours to eat dinner when you were on holiday?

    I know you mention that you’ve had CBT in the past but I really do think you would benefit from some more psychological input with this issue, there maybe a different way they can help you to approach this.

    It can be truly exhausting when your life has to revolve around these sorts of rituals and I agree that there are some OCD traits within your behaviours that you’ve mentioned. You have adjusted quiet a lot of your behaviour already but when it’s taking over in the direction it is currently that isn’t helpful to your wellbeing long term.

    Another vote for a slow cooker here.  I do suspect there are some psychological problems with eating, and possibly with routine, but think a slow cooker might be a good way to experiment with these, whilst having the security that a meal (and one which will not spoil if you are delayed!) is waiting for you at the time required.

    I am more worried there may be an undiagnosed physical problem

    I definitely don't need to lose weight. My problem has always been being too skinny. People think I'm a light eater from looking at me. I've had people say "where are you putting all that food?" many times when they've seen me eat.
    I don't mean to be alarmist or overly personal but this is a bit concerning. 500g mince would do 4 adult portions of spag bol for me/my husband (and he's not a light eater). How many calories are you having per day? If you're eating more than the recommended amount of calories and are still "too skinny" it may be worth talking to a doctor. Yor body may not be absorbing your food correctly. Sorry, I know you didn't ask for medical advice and this is obviously something that is stressful for you. I hope things get better for you.

    Consider CBT again, you need to be in the right place for it.  It was only on the 3rd attempt that it "clicked" for me.

    If you're eating as much as you are and you are skinny, there still may be a problem.
