Help with getting a dropped curb?

Where I live, house prices are massively affected by parking. I have read my local council guidelines and believe I'd qualify for dropping the curb, however the application process states:
These items are mandatory: Site Location Plan Existing Plan Proposed Plan Application Fee.
I am a bit unsure about what is needed for the first three boxes. Does anyone have any idea or have you had a successful application?

Site location plan = map with a DOT to show where your house is in relation to other houses and distance from corners/bends etc.
Existing plan = a drawing showing your house (in close up) with your drive and kerb as it exists now.
Proposed plan = as above but with the dropped kerb shown
Application Fee = Money (Oh sorry you know that one!)1 -

It may be worth searching for similar applications on your local authority's planning portal to give you an idea of what to supply.0

Possibly print a screenshot from Google maps and mark the position of your property on that. Show the position of any existing drop kerbs and where you want to add one.

No - you don't need a professional for the proposed plan. Just a sketch drawing with a floor plan view of the drive to be should do.
You will however need a professional to get the lowering of the kerb done and must have insurance. Your council will provide a list of preferred contracts with your approval.
