    You can setup an Apple ID without a payment method but that’s not what the OP did. 

    If the child wants to then buy content they can top up their account with iTunes gift cards. 
    This!!! I've (nearly - Amazon is a little more difficult) always set up accounts for other people (family - whether adults or children), whether it be Google/Microsoft/Apple/Leapfrog without a payment method attached. If apps are required, then its a case of buying the appropriate gift card and topping the account up that way - yes there's often a little disparity between the gift amount and purchase, but I'd rather have the extra pence as future credit, than attach my actual credit card. Also if you keep your eyes open there are often offers for available from legit retailers to buy cards for less than face value.

    Not if you never check your bank or credit card statements you wouldn't. And then, even if you did, it was only 4 debits showing on each statement, over 12 months. Tricky to spot, obviously.
    No free lunch, and no free laptop ;)

    My 3 year old son once bought a bloody sofa on sims for £80. Apple was actually very helpful and gave me a refund. No harm in asking
