Been told i need a "Fit Note & to claim universal credit

I have various medical problem i have aspbergers pernicious anaemia and a umbilical hernia the size of a football.
I have been on Jsa for many years i used to be a gardener but am now unable to work would i be better off on universal credit and getting a fit note 
  i currently get jsa with a disability premium and pip high rate mobility and care

    Do you receive the severe disability premium in with your JSA or as part of a housing benefit claim because you're living alone or classed as living alone an no one claims carers allowance for looking after you? If so then you won't be able to claim UC and it will be ESA that you'll need to claim with a fit note from your GP.

    If you don't receive the SDP then you will need to claim UC with a fit note from your GP.

    If you are unable to work it is unlikely you will be allowed to remain on JSA indefinitely.
    Are you living alone? When you say 'disability premium', is this just the Disability Premium or is it the Severe Disability Premium (SDP)?
    If you get the SDP you are not allowed to claim UC but can claim the old form of ESA.
    Information I post is for England unless otherwise stated. Some rules may be different in other parts of UK.
