    That's correct. If it was me then i'd also send a copy to the health assessment providers, just in case your form is sent there as soon as it arrives at DWP.
    OK thanks I'll call them up on Monday and see what they advise

    Had some extra info to send to the DWP and the address she gave me over the phone was different from last time. Is this because they want you to send it to whichever place is less over loaded? 

    If I recall correctly, when I wanted to send some more information regarding my claim they said site B as opposed to the previous site A or vice versa
