Just sent off my personal independence form. Want to contact the DWP so they inform me once they receive it, what's the best way of doing this? Thank you

The number on the letter you got originally for contacting them, but give it a few days to be put on the system,1

If you added a mobile number to the form they will send a text once it's received and added to the computer.

I sent a letter from my psychiatrist for supporting information however I'm not 100% sure it had the address for the clinic on it, there was a page attached to the psychiatrist letter with the phone number and address which I didn't send, is this important? I did write the number and address in the form0 -

As you wrote it on the form then that's fine. Although they rarely contact anyone for any evidence.

I'll send it off just for good measure and peace of mind.
All I need to do is write my national insurance number and name etc, then send it off first class?0 -

That's correct. If it was me then i'd also send a copy to the health assessment providers, just in case your form is sent there as soon as it arrives at DWP.
