OK yes I gathered that from the form
I've given lots of details throughout especially on the last page which asks for extra information
Hopefully I can get this letter this week and send it off ASAP
The clinic is in the next town and usually post from them comes the next day so fingers crossed it will be ok0 -
Remember that you can send extra information if it becomes available after you have returned the form. At the moment in my area we are seeing over three month delays between return of PIP form and IAS contacting claimants for face to face appointments.Information I post is for England unless otherwise stated. Some rules may be different in other parts of UK.0

It isn't just based on a diagnosis, no, but if you're claiming on the basis of, 'I have this condition and it affects me like this', having proof that yes you do indeed have that condition is always useful
