Help With Personal Independence Form

Never filled out the forms before as I used to have an advocate.


Received it today and just wondering, if things have stayed the same do i put " Stayed the same " in the top box, then in the middle box explain how I manage the tasks and in the bottom box put " stayed the same "

    I wouldn't advise to just write no change. All claims should be treated as a new claim by all claimants and you should put as much information as possible, then add a couple of real life examples of what happened the last time you attempted that activity for each descriptor that applies to you.


    There's never enough of room to put all the information, so use extra sheets of paper but made sure you add your NI number and name to everything.



    Send relevant evidence with the form to support your claim.

    On the final checklist it says "3. Included photocopies of supporting information with this form"

    I don't have any photocopies but I have left details of my cpn and my psychiatrist is that OK?

    Not sending evidence with the form is not advised because they very rarely contact anyone for this. The onus is on you to make sure it's sent to support your claim. If you need to get copies of evidence then you can request 14 days extension to return the form and hopefully this will give you time to get some photo copies.

    If you know or can find out (ask your previous advocate) what evidence was sent with your previous form, send the same again if you have nothing new. It's better than nothing.

    So I need evidence of my mental health diagnosis (paranoid schizophrenia}??

    I'm not sure what we sent before as my advocate did it for me

    What I can do is call my cpn and ask them if they can assist
    I have until 16th February according to the form

    It says " send us any information that you've received since you last award"
    I haven't received anything like this though?

    they suggest you send evidence of the disability to assist with the claim
    I.E a recent report from an O. T
    A repeat prescription list
    A recent care plan
    Or a carers diary or letter

    So I'll ask my carer or cpn this week then :money:

    OK I was told by my care team to get a letter from my psychiatrist and send that off with the form

    It will certainly help. However what you have to convey to PIP is how your health affects your ability to carry out the activities. Different people with the same health condition can be affected differently.
