it's been a little while since anything has happened but a couple of weeks ago we received a "general form of judgement or order" saying the judge has allocated it to the small claims track, and the parties are to produce written statements of evidence and witnesses "to arrive no later than 14 days before the hearing"; but we don't have a hearing date on the letter or any mention of one on the MCOL site? When do they require them by?

Contact the court that produced the order and ask them. It might be a genuine oversight but you do not want another "general form of judgment or order" because you didn't file and serve WS in time.2

MCOL - you do not use MCOL at any point now. That time has passed.
As above. We cant tell you, only the court can. It could be they havent adjusted their templates for the covid crisis.3 -
