Not in my experience since the mid 1980s when merchant narratives were indeed not often transmitted accurately and replaced by generic wording such as 'Hotel 4 Estrellas' which would be transmitted for any 4-star hotel in Spain. Even that would not be generated by the card issuer.
The only realistic 'doctoring' of the merchant narrative by a card issuer would probably extend no further than truncation of some of the characters - but, even that is not going to be commonplace (if at all).
