Could be - however, the CC supplying bank could just display a different narrative (that they've generated themselves) on the statement.

Not in my experience since the mid 1980s when merchant narratives were indeed not often transmitted accurately and replaced by generic wording such as 'Hotel 4 Estrellas' which would be transmitted for any 4-star hotel in Spain. Even that would not be generated by the card issuer.

The only realistic 'doctoring' of the merchant narrative by a card issuer would probably extend no further than truncation of some of the characters - but, even that is not going to be commonplace (if at all).
