    Dear Bank
This is a Subject Access Request for all info you may still hold on me
    My addresses were
    ......
    .....
    My bank account numbers were
    ......
    .....
    Thank you very much
    Mar.

Your bank account isn't pertinent to a PPI complaint, your credit card and loans are. Just because they came out of your account, doesn't mean there's any PPI info contained in those DDs. And if you want to complain about a packaged account, that's another different thing.
    Another letter with
    Dear Bank
I believe I was missold a packaged bank account because......

If you have closed this account in the last six years, they may time bar you. Depends if you still bak with them or not, whether the account was closed, whether you used any of the benefits.

I can't actually believe that the departments cant have some kind of database where its all available to every department etc :/

    a) data protection act 1994 recently replaced with GDPR.
    b) computerisation was limited until the 2000s and even then it was limited. So a lot of records are microfiche, if they were copied at all.

Wow, ok il go and ask elsewhere for advice on how to put particular info into the SAR
"Elsewhere" should be the Bank directly, as I've already told you. Whether you send this SAR online or by post, they still have forty days in which to respond. This is a timescale they will adhere to.
Do be aware that they've already told you they cannot find any PPI...

As with the previous reply, the fact that you have complained about a packaged account (which they acknowledge existed) has no bearing on whether they have also kept records of any PPI you may (or may not) have held.
