How to claim back charges/fees?

Hi I did a balance transfer to Asda Money credit card in January at 0% with a 3% fee. I looked at my Feb statement and it said the min payment would be taken. Good I thought. Everything ok. Until I had a credit file warning via clearscore.
I have been busy working, been overseas for work, and just found out I have been charged a default fee of 12.50, but worse had the 0% offer rescinded and now I am on a standard balance rate - and no refund of the 3% fee! I asked why and they said I would be taken to court for non payment.... I checked my statements no direct debit. Asda said they cancelled the direct debit! I immediately paid the outstanding balance and complained. A week later I have heard nothing and basically they seem to be saying "go away".
What are my best legal options? I have a copy of my statement saying the direct debit was in place fro February 2019 and online also a screen shot saying it was in place (Asda claim they wrote to me saying they had cancelled it - I can't find any such letter) and want my 3% back of the deal back. Small claims court? Financial ombudsman?
Of course this has seriously impacted my credit file.

I would complain in writing, to Asda in the first instance. Include your supporting evidence with your complaint. (ie. the statement that showed the DD in place and the screenshot too). Also mention that you never received a letter from Asda informing you that the DD was cancelled by them.
Only if the outcome is unsatisfactory would I escalate the complaint to the FOS.I work within the voluntary sector, supporting vulnerable people to rebuild their lives.

And this was there initial customer service response, or the resut of a protracted argument ?

I have complained in writing, heard nothing back and customer service is basically non-existent. I presume then I wait 2 weeks than Financial Ombudsman complaint?
The reason they gave for cancelling the DD was "lack of usage of account" but they seem to have very bad systems as the statement says it was in force.
Anyone considering ASDA Money beware!0
