Roundabout accident - who’s at fault?

This is 'straight on' at a 2 lane in, 2 lane out roundabout, and on Street View, I can't see any road markings giving any direction. So OP was within his right to expect to be able to go straight on, but he would have been in conflict with the lady, if she was indicating right to stay on. She shouldn't really do that, and I'd 'blame' her, but OP is crossing the lane on the roundabout, so should be watching out for this very thing.
Here's the only specific signage for the roundabout. I'd be in the left lane, unless I knew there were 2 lanes at the exit, and wanted to overtake. But I'd be careful!

From that signage, there is no "straight on", so according to the Highway Code the OP should have taken the LH lane.
BUT, this is Northern Ireland, where the HC does not apply, and the rules may be different.0 -
I think the worse part is where you (as the black line) mounted the roundabout on the way round...(just joking)

Though, if you follow the streetview the 'boost' liveried VW van in front does the same thing....As does the Renault behind0 -
Highway Code applies in Northern Ireland last time I checked! There is a 'straight on' - it's at roughly 11/12 o'clock. Not perfect, but close enough to apply the rule, IMO.0

There are two lanes leaving the roundabout, so clearly two lanes of traffic can leave the roundabout at once. However the road markings on the roundabout go around forever, making the person on the left think both lanes are their perogative.
